Identifying as a carer

If you provide unpaid care for a family member or friend due to illness, disability, mental health issues or substance misuse, it is important to identify yourself as a carer.

By doing so, you can access a range of support services designed to assist you in your caring role.

Registering as a carer with the practice

We maintain a confidential register of carers to ensure you receive appropriate support and information.

To register:

  • Contact us: inform our reception staff of your role as a carer
  • Complete a registration form: you may be asked to fill out a simple form to provide relevant details about your caring responsibilities.
Support available

Once registered, you can access various forms of support:

  • Information and resources: receive updates on local and national support services, events and resources tailored for carers.
  • Carer’s Needs Assessment: we can refer you to Social Services for an assessment, which evaluates your needs and identifies support to make your caring role more manageable.
  • Health and wellbeing support: access services aimed at maintaining your health, including mental health resources and respite care options.

There are lots of organisations that can help you if you are a young carer or young adult carer.

Carers Trust is one of them. Carers Trust runs a network of local carer organisations in England, Scotland and Wales.

They offer information, advice, practical support and care in the home to unpaid carers of all ages. Many local carer organisations in their network have dedicated services for young carers and young adult carers.
