New patient registration

Patients who wish to register with the practice will be expected to live within the practice boundary.

How to register

You can register by downloading our MySurgery app – this makes things quick and simple.

Please watch the video demonstration of its benefits then download on the links below.

Alternatively, you can visit the Surgery and collect the appropriate forms. It may be necessary to be seen by the healthcare assistant, who will complete your medical history questionnaire.

Change of personal details

Change of personal detail forms are available on our website.

We need to know of any changes including your name, address, telephone number and mobile number as it is important to have your up-to-date details to contact you or refer you to see a hospital consultant.

If you move outside the practice area you will need to register with a doctor in the area you have moved to.

Please note that we do not register patients outside our practice boundary.

 

Change of personal detail forms are available at reception in both surgeries.

Online practice 'pre-registration'

If you wish to pre-register, click on the link below to open the form. When you have completed all the details, click on the “Send” button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

Data protection

Note that by sending the form you will be transmitting information about yourself across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect

Alternatively, you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.

Temporary Registration

Please note that our list is temporarily closed due to GP Recruitment issues

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and remain a patient of your permanent GP.

After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

New Patient Registration

DURING CORONAVIRUS (COVID-19) PERIOD

We welcome new patients.

We require all new patients to complete a registration form (GMS1W), and a health questionnaire (below) which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. Both forms can be found below.  We will accept two forms of identification that prove you live within the area, photo and one with proof of address. Your NHS number is also required to register, this can be obtained from your previous surgery. Please also supply a copy of any repeat medications from your previous GP to ensure we have proof of medication. 

Please complete and attach using the online forms below. We will accept a photograph of the evidence; if you can not access a printer/scanner. Please note if you do not complete all information required or attach proof of address this will delay your registration.

For patient safety and confidentiality; please note by returning a completed form you are signifying you are who you say you are and all information is correct to the best of your knowledge. This will mean you also give permission for us to set you up as a New Permanent Patient at our Practice.
